Medical officers of all primary health centers have been given the responsibility of this survey. This is a highly specialized survey, with red, green, yellow and silver cards being distributed to gram panchayats based on their criteria and risk classification of sources.

A gram panchayat is given a red card if more than 70 percent of its population depends on high risk sources and a green card if more than 70 percent of its population depends on low risk sources.

If more than 70 percent of the population in a gram panchayat does not depend on high risk or low risk sources, it gives yellow card to the gram panchayat. In group gram panchayats, if the above findings are found in a village, yellow card is given to that gram panchayat by adding the name of the village concerned. Silver card is given to Gram Panchayats that have not had an outbreak of epidemic for five consecutive years and have received green card for five years.
